Have you looked at the "municipal cinemas"?
They tend to focus on "culturally important" stuff though, so don't expect box office there.
In Heidelberg, that'd be the Karlstorkino, which shows pretty much only subtitled movies in original language. English, French, Spanish, Portuguese, Farsi, you name it, they got something.
Location is (duh) "Am Karlstor 1". It's the old Karlstor station.
In Mannheim, it's the Cinema Quadrat, located in the huge Collini-Center (somewhere on the ground floor on the south side of the Neckar, can be hard to find). Their programming is horrible to navigate, some english-language movies up in the next two weeks would be "The Navigators" (UK, 2001), and "Three Burials" (2006, by/with Tommy Lee Jones). German Subtitled.
A few other cinemas in Heidelberg occasionally to rarely show original-language (including english) or subtitled movies. Offhand, in Heidelberg, that's mainly Gloria (in Hauptstraße), Kamera (Neuenheim) and Studio Europa (Weststadt). All of these are usually rated "Arthouse" movie theaters (and only have a single theater, except Gloria), although Studio Europa for example also shows pretty mainstream box office stuff.
